Dr. Viera’s patients often ask what areas are approached during facelift surgery. He finds that this is easier to explain if the face is divided into the three known aesthetic areas: The upper, the middle and the lower third areas:
- The Upper Third is encompassed by the forehead. This area is not treated during a facelift. It is treated during a separate procedure called a Browlift.
- The Middle Third extends from below the eyebrows to below the nose and encompasses the nose, eyelids and cheeks (mid face). The midface is lifted during our Lifestyle™ facelift as an independent unit, this requires a surgeon well versed in facial plastic surgery, most particularly one who is Board Certified in Facial Plastic Surgery
- The Lower Third extends from below the nose to below the chin and includes the mouth lines and jowls as well as the neck.
Facelift surgery, in most surgeons’ consideration, encompasses the lower third area. Again, this is the area below the nose, including the mouth lines and jowls. Most surgeons will approach the jowls and the neck to various extents. Few will raise the mid-face (middle face or cheeks). Doing so improves the mouth lines and jowls more effectively. It raises the cheeks rejuvenating this area of the face, an area which sags with aging.
The mid-face area, or the cheeks, has been neglected in the past. This area can be lifted during a traditional facelit. To do so requires that the cheek mound be lifted as an independent unit during the facelift procedure. If you were to pull up and to the side on your cheeks, you can notice that doing so exerts a greater pull on the mouth lines than solely pulling on the lower face alone. To do so, however, requires a surgeon well versed in facial surgery, most particularly one who is Board Certified in Facial Plastic Surgery. Most facelift surgeries performed in the past and even today do not include the mid-face since it requires expertise in facial surgery as possessed by Board Certified Facial Plastic surgeons.
How long will the ActiveLift™ Facelift last?
The duration of the ActiveLift™ facelift will depend on several important factors:
Puregraft™ is currently the most advanced fat transfer technology. Fat augmentation has, up to this point, consisted of directly injecting the fat immediately after removal with little success. Impurities were not filtered from the fat before the transfer. Puregraft ™ filters the fat and results in healthier, more viable fat cells which practically eliminates inflammation. Previously, the redness and swelling of inflammation interfered with the fat’s viability and longevity.
Puregraft™ allows us to be confident that the fat we transfer is viable and long-lasting. Procedures such as injectables or implants will be replaced by transferring your own fat from where you don’t want it to where you do. Rather than wasting the fat removed during liposuction, this technology allows us to re-use the fat. Puregraft ™ can be used to make plump, soft lips, decrease the “smile lines” and “marionette lines” around your mouth, as well as many other areas. What could be more natural than using your own fat?
1. How old is the person seeking the procedure?
As we age the skin tends to lose collagen, elastic fibers and fat, another words it atrophies becoming thinner and losses elasticity or snap-back. Therefore it will stretch and sag soon thereafter. “I recently saw a close picture of Camilla, Prince Charles’ wife. She had a facelift performed prior to their wedding. I could tell that her neck and jowl sagging had returned five years later.” – Dr. Viera
2. What is their lifestyle?
Certainly a person who smokes or drinks excessively as well as leads a stressful lifestyle cannot expect a long lasting result. Dr. Viera would certainly inform that person he or she is not a candidate.
3. The type of facelift performed
A person in their early to mid forties will need a much lesser facelift than someone in their sixties to seventies. Younger people are candidates for one of the various types of mini-lifts. A mini lift provides less lift, since less is required and threrefore less surgery (less invasive). Today, mini-lifts have been popularized and many surgeons have given a name to what is still basically a mini lift, making it appear they have invented the wheel when in fact this was the first type of lift performed back in 1940. The incision for this lift is limited to and hidden within the ear lobe.
Someone in her (his) sixties to seventies will need a more extensive lift so as to include a neck lift as well as a cheek lift. Some fat liposculpturing will likely be needed in the neck and the jowl area as well as fat transfer around the eyes and mouth lines. The cheeks may have flattened so fat may be transferred there also.
In our practice we want all of our patients to look natural; like themselves but with a soft, more youthful look.
What care is exercised after recovery?
Just as you need to care for and maintain your car, you must provide care for and protection of your skin. Again, good habits, avoid liquor, sun and smoking as well as stress (good luck). Also good cleansing and moisturizers (here the least expensive are better, aloe vera etc).
Qualifications of the surgeon performing the surgery.
Last and most important, the surgeon should be Board Certified in Facial Plastic Surgery. This is indicative that the surgeon has been verified by the Board to have extensive facial plastic surgery experience and passed rigorous testing in this area.
Our Activelift™ was designed as a minimally invasive facelift but yet it will approach all the areas mentioned, it raises the mid-face (cheeks) as well as the neck and jowls. Areas with fat accumulation, such as the jowl and under the chin, are liposuctioned. Pulling is exerted on the underlying musculature so as not to have any pull on the skin. This provides for a very long lasting lift and avoids a windswept look which occurs when too much pull is exerted on the skin. Incisions are hidden in the temple hairline.
